By completing this form you agree to the following Parent/Guardian permissions:

-I accept that POOSH  educators are happy to remind my child(ren) to attend the extra curricular activity but will not be held responsible if my child does not go when reminded.
-I accept that my child attending and walking to and from activities during an POOSH session will not be under the supervision of POOSH educators.
-I understand that POOSH educators are not responsible for my child whilst they are absent from  POOSH
I accept that if I collect my child directly from the extra-curricular activity when they would usually return to POOSH I need to notify the Centre that they will not be returning.
-If my child is not returning to the service after the activity I give permission for  an educator at the Poosh Centre to sign my child out of the service via Hubworks sign out (r99b).
- I understand that it is MY RESPONSIBILTY to organise with the activity coordinator the arrangement for returning the child to Poosh. This is not Poosh’s responsibility.
- I understand that if my child is returning to Poosh and does not return at the specified time, Poosh will search for my child, try to locate the activity coordinator. By 10 minutes after the expected arrival time Poosh will ring an authorised parent/guardian to notify them of the situation.
