Unplanned Capital Repair Form
An Unplanned Capital Repair is either:
• An emergency repair that, without correction, prevents a facility from safely being used for educational purposes. An emergency repair would create a facility with no heating or cooling, no sanitation, water intrusion that halts the continuation of educational programming or health and safety code violations. Funds for an emergency repair could also include approved insurance deductibles; or
• A repair or replacement that is not identified in a school’s Facility Condition Assessment Report but meets the criteria of a capital repair or replacement Priority. For the 2022-2023 school year, only Priorities 8, 9 and 10 are considered for Unplanned Capital Repairs; or
Using the definition above, if Charter believes their repair qualifies as an Unplanned Capital Repair, Charter must fill out this form with all required fields. Once received, NOLA-PS will follow up with additional required documentation and next steps.
