COVID 19 LATERAL FLOW TESTING CONSENT - ALL STUDENTS

Ormiston Six Villages Academy will follow the guidance set out by the government and NHS, using test kits provided. As a parent/carer, you will be consenting/not consenting to the following:

1. For your child to have lateral flow test(s)
2. For Ormiston Six Villages Academy to keep a record of the test results in line with government requirements.

Yes - I consent to the regular Covid 19 lateral flow testing

No - I do not consent to the regular Covid 19 lateral flow testing. Please be aware the DfE and the NHS have advised us that “people who decline to participate in serial testing will follow the usual national guidelines and are legally obliged to self-isolate following contact with a positive case of Covid-19 according to the advice given to them by the NHS Test and Trace service”.
