By signing this form, I acknowledge that I have read the Doulaing The Doula Birth Doula Standards of Practice and the Code of Ethics. I also affirm that I am agreeing to follow the guidelines in each of these documents. By signing this agreement, any births I attend will be eligible for DTD certification if I should choose to join their certification program. Both Doulaing The Doula and I understand that this agreement is not legally binding. However we both consider it to be ethically binding.
