To abide by the current restrictions and regulations as outlined by the Government of Ontario and/or Ontario Public Health as well as the COVID-19 safety protocols of Fair Havens Ministries.

That I/we have not been advised by any government authority to isolate due to possible exposure to COVID-19.

That I will advise Fair Havens Ministries immediately if any of the Attendees has been exposed to COVID-19, has contracted COVID-19, or is showing any of the above-listed symptoms prior to our arrival or during our stay at Fair Havens Ministries.

I/We voluntarily agree to assume the risk with respect to COVID-19 inherent in my attendance at Fair Havens Ministries and participation in the activities of Fair Havens Ministries.

Activity Waiver and Condition of Participation

The Attendees hereby release Fair Havens Bible Conference, Associated Gospel Churches, Fair Havens Ministries, as well as its agencies, directors, staff and agents from liability as a result of any loss, personal injury, sickness, accident, misfortune or damage done to the named participant or his/her property, with the understanding that reasonable due diligence shall be taken to ensure health and safety of the participant. Activities may include but are not limited to:

• Canoeing, kayaking, paddle boarding, and other non-motorized water activities.

• All other activities and programs as may take place from time to time, including but not limited to soccer, baseball, basketball, tennis, pickleball, biking and other activities.

• All kids and youth ministry activities.

The Attendees and/or parent/guardian certify that the Attendees are in good health, normal in condition and habits, and are amenable to rigorous activities. By signing this form, the Attendees and/or parent/ guardian gives the staff of Fair Havens Bible Conference and its agencies the right to obtain medical attention/ treatment deemed necessary for the Attendees’ welfare and good health. In such a situation, effort will be made to contact the emergency contact and/or parent/guardian as soon as possible. The participant or parent/guardian is responsible for all costs incurred.

The participant and/ or parent/guardian give permission to use any photography or video of the participant for promotional use.

Fair Havens Bible Conference Ground Rules & Regulations

In order for Fair Havens Bible Conference to maintain an enjoyable experience for all, we ask that you acknowledge the following:

• Pets are not permitted on the grounds. Our office would be pleased to recommend some local kennels/boarding services.

• Fair Havens maintains an alcohol, smoke and vape free environment. Non-medicinal drugs are not permitted on the grounds.

• Guests are required to wear a wristband at all times as proof of payment to use any and all facility amenities. Wristbands for all attendees will be provided at check-in. Silicone wristbands will be available for purchase, if preferred at the office.

• Cooking of any type is not allowed inside any of our guest rooms or cottages that are not equipped with a kitchenette.

• As a family-centered camp, we encourage modesty in all attire and require that no revealing bathing suits be worn ages 13 and up at our beach, splash park or pool area.

• All individuals 18 years and under must wear helmets while riding on grounds. Lights on bikes are required after dark.

• You are encouraged to participate in chapel/children/youth ministry activities as often as possible during your stay.

• Campfires are permitted in designated Fair Havens Ministries campfire pits. If announced by our region, fire ban notices will be posted and must be adhered to. Firewood bags can be purchased at the office during office hours.

• Ice can be purchased at the office during office hours.

• Garbage and recycling disposal is located by the Quonset. Please do not leave garbage on camp sites overnight as it will attract animals.

• Please keep campsites neat and tidy, and respect the guests beside you. Each tent and trailer site has one picnic table and campfire pit for use during your stay.

• A designated dumping station for sewage and gray water is available for trailers. Please do not release on campsite.
