Please ensure all name spelling and performer/character categories are correct.

This content of this form must be verified by two representatives of the society - one, a member of the Executive Committee and the other, the Director of the production.

It must be completed and received two weeks before opening night.

Please note: in order to be eligible for NAPTA Judging, the show must be on the NAPTA/Zone 1 Calendar. If it is not in the calendar, and opening night is less than 2 months away, it may be added to the judging schedule at the discretion on the Convenor and Judging Panel.
