2024 Mathematics Commencement Special Accommodations Request Form
We are pleased to welcome you to the 2024 Mathematics Commencement Ceremony! In order to make sure all of our guests' needs are met, we are using the form below to receive more information on accommodations you will need to enjoy the ceremony and celebrate our graduates. Please submit this request form no later than Monday, April 22nd so that we have ample time to make arrangements. We will do our best to accommodate requests made after this deadline, but our ability to support guests may be limited depending on the accommodation required. 

Some examples of accommodations include special seating for guests who cannot manage stairs, communications accommodations, CART services, or ASL Support, etc. Please note that for each individual needing special seating, a maximum of one additional guest per, may accompany them in the reserved seating section.
