Devotee Screening Questionnaire for In Temple Private Priest Service
IMPORTANT INFORMATION - PLEASE READ

Every member of the household must submit this form individually, including children who are planning to accompany you.

IN TEMPLE PRIVATE PRIEST SERVICE
1. Devotees are required to call Priest to schedule appointment for "In Temple Private Pooja Services" [Pavan Kumar:309-825-8438]
2. Devotees are required to submit this form not more than 3 days prior to the Pooja date. All family members planning to visit the temple for Pooja are required to submit the form individually (One form per family NOT allowed). No guests allowed; Up to 6 people allowed from the sponsor's family.
3. Devotees are required to bring all the Pooja Samagri as told by the priest. We request you to purchase Samagri items 2 to 3 days prior to the Pooja, so as to naturally disinfect the items.
4. Homemade Naivedyam NOT allowed
5. Devotees to take back all remaining items, clean up the area and disinfect. (Disinfectant Spray bottle will be available)
6. Mask must be worn at all times and 6 feet social distancing must be maintained. The Temple reserves the right to refuse services to devotees that fail to adhere to social distancing standards.
7. Dakshina to priest should be given in an envelope.
8. Do not bring any items to be given to priests as Daanam. In lieu of items, you are welcome to make a monetary Daanam to the priest.
10. Payment for the Pooja Service can be made at the temple on the day of the Pooja.

VAHANA POOJA
1. Devotees are required to call the priest and reserve an appointment for Vahana Pooja.
2.After scheduling an appointment, but not more than 3 days before the Vahana Pooja, fill out and submit this screening questionnaire. All family members planning to come in for the Vahana Pooja are required to submit the form individually (One form per family NOT allowed).
3. Devotees are required to bring all the Pooja Samagri as told by the priest. We request you to purchase Samagri items 2 to 3 days prior to the Pooja, so as to naturally disinfect the items.
4. Bring your car to the "Vahana Pooja Designated Area" (on the South side parking lot, near the mobile storage) at your appointment time.
5. Call the priest to notify your arrival [Pavan Kumar: 309-825-8438]
