Lash N'Learn (Lash Extensions Training) Application
GROUP CLASSES: Thank you for your interest in Lush Lash Academy's "Lash N' Learn" Classic Style Eyelash Extensions Class. Eyelash extensions are one of the fastest growing beauty services and provide a lucrative beauty career for certified and licensed lash extension artists.   At Lush Lash Academy, we offer in-depth training and courses designed to teach and educate beauty professionals in the art, safety, and application of eyelash extensions. This semi-permanent individual eyelash extension training (applying 1 extension on 1 natural lash) is an intense 2-day course that will educate you on all the important aspects of applying eyelash extensions properly. This is an extremely “Hands ON” training requiring you to supply your own live model for day 2.

Upon completion of this online application, we will contact you with information to pay and confirm your spot in our eyelash extension training class, if you have not done so already.  Your deposit is required to reserve your spot in our group class.  A partial payment deposit is required to reserve the day(s) for your individual private lesson. PRIVATE ONE-ON-ONE CLASSES: Our one-on-one classes are designed to teach everything in our general group class with the additional individual attention that a student may need, along with the flexibility in scheduling.  We offer a 2 or 3-day program for students who wish to learn more in depth instruction in lash mapping and gain additional time with live model hands-on lash application under the supervision of our expert instructors.

Paying the class fee balance: A payment link will be emailed to you 10 days before the class date.  The balance for your class fee + any fees for a starter kit must be paid within 3 days ( 7 days before class).  - Please type "I understand" below. *

Please read the following policy:
Once the registration form is completed, you will receive an invoice for your deposit of $350, if you have not already paid a deposit online. You must pay this deposit within 48 hours of submitting your application, and a full 10 days before your class. The balance of your class fee will be due 7 days prior to the start of your class.

REFUND POLICY
All payments to Lush Lash Academy are subject to the following Terms and Conditions.
* Deposit  $350 is non refundable if refund is requested less than 21 days prior to class date.  If refund is requested with at least 21 days notice prior to class date, a $150 administrative fee will be deducted from the $350 deposit and a total of $200 will be refunded within 30 days.

FOR FULL PAYMENT REFUNDS
If you have paid for the class in full at least 30 days prior to class date and are requesting a refund within 21 days prior to class you will receive a full refund less $150

For all payment plans:
*Cancel 7 days before the starting date of class – NO REFUND
*Absence (No Show)  No Refund
*Re-schedule fee of $100.00

Live Model Requirement:
You MUST provide your own live model for certification portion of the class.  If you do not provide a live model, you will not receive the certificate and will need to take the re-certification course at a time when you can provide a live model, and pay a fee of $350 for the additional re-certification portion
