We are delighted you have decided to schedule an appointment with Dr. Lalezar at the Health and Vitality Center. These are a few points to keep in mind prior to your upcoming visit.

Forms
Please make sure to fill out the forms prior to your visit. Forms can be downloaded on our website under contact us. Please arrive 15 minutes prior to your appointment.

Fasting
Please fast for 8 hours (you can drink water) prior to your first consult if you have not had blood work within the last year. Feel free to bring a snack to have after your blood draw. We have fresh organic fruits, protein bars, and hot tea in the office. Please don’t take thyroid medication on the day of the blood test.

Location and Parking
Our office is at 11600 Wilshire Blvd Suite 120, Los Angeles, CA 90025. We are located on the street level next to Horton & Converse Pharmacy.

We are west of the 405 freeway on the southwest corner of Wilshire and Federal. There is street parking on Federal that is free (no meters). Please note street cleaning is scheduled between 10:00 A.M. and 12:00 P.M. on Thursdays and Fridays. There is also a parking lot off of Federal, which charges $3 every 15 minutes with a maximum of $27. Please note we DO NOT validate parking.

Billing
Please note we are out of network with all insurances. If you have PPO insurance, we can only provide you with a superbill for the consultation fee. If blood work is being performed, we will make sure to submit your current insurance information to the laboratory. Some specialty laboratories are out of network as well. We will get your consent prior to performing those labs.

Cancelation Policy
You will receive a confirmation phone call or text message 48 hours prior to your appointment. If you need to cancel or reschedule, please call our office 48 hours prior to your appointment.
Failure to reschedule or cancel within 48 hours will incur the following fees:
