Hello! Please fill out the following form to register for our 2021 Summer Reading Program.  Only complete this form when initially registering for the program. If you are a parent registering multiple children, you will have to fill the form out for each child. If you are a parent filling out the form for a child or teen, don't forget to register yourself for our adult summer reading program as well!
