By completing and submitting the form above, you hereby acknowledge and agree that Renew Community Church (the “Church”) provides the personal coaching services (“Services”) for your personal educational purposes only. The Services are in no way to be construed or substituted as psychological counseling or any other type of therapy or medical advice. The Released Parties (defined below) cannot and do not guarantee the outcomes of the Services. Further, you hereby acknowledge and agree that the Church and its affiliates, successors, and assigns, and their respective directors, officers, employees, and agents (“Released Parties”) do not assume and will have no liability or responsibility to you or any other person or entity for the Services or any results or actions taken by you as a result of the Services. You hereby acknowledge and agree that your use of the Services is entirely at your own risk. You hereby acknowledge and agree that by using the Services, you may be engaging in activities that involve the risks that may result from the actions, negligence and failure to act of yourself and others. You should consult your counselor, therapist, or other or other health care professional to determine if it the Services and any recommendations contained in them are right for your needs. YOU KNOWINGLY AND VOLUNTARILY ASSUME ALL RISKS ASSOCIATED WITH THE SERVICES, INCLUDING, WITHOUT LIMITATION, PERSONAL INJURY, DISABILITY, DISMEMBERMENT AND DEATH), ILLNESS, DAMAGE, LOSS, CLAIM, LIABILITY, OR EXPENSE, OF ANY KIND OR NATURE, THAT YOU MAY SUFFER ARISING OUT OF OR IN CONNECTION WITH THE SERVICES (“CLAIMS”). WITHOUT LIMITING THE GENERALITY OF THE FOREGOING, ON YOUR OWN BEHALF, AND ON BEHALF OF YOUR HEIRS, EXECUTORS, ADMINISTRATORS AND NEXT OF KIN, YOU HEREBY RELEASE, COVENANT NOT TO SUE, AND FOREVER DISCHARGE THE RELEASED PARTIES OF AND FROM ALL CLAIMS, AND FURTHER AGREE TO INDEMNIFY AND HOLD EACH OF THE RELEASED PARTIES HARMLESS FROM AND AGAINST ANY AND ALL SUCH CLAIMS INCLUDING, BUT NOT LIMITED TO, ALL ATTORNEYS' FEES AND DISBURSEMENTS UP THROUGH AND INCLUDING ANY APPEAL.
