If you are in need of assistance with Thanksgiving, please fill out this form completely, no later than September 20th! NO LATE FORMS WILL BE ACCEPTED!
All Thanksgiving assistance will be done through God’s pantry, and you are responsible to pick up your own Thanksgiving basket.  If you need assistance with transportation, please contact Mrs. Swauger.  

The Thanksgiving basket contains (may be subject to change based on availability).
Turkey                Instant Mashed Potatoes
Canned Vegetables Gravy
Canned Yams        Canned Fruit
Cranberry Sauce        Dressing Mix
Corn Muffin Mix        Dessert
Margarine        Eggs & Onion

Distribution of Thanksgiving Baskets will take place at Southland Christian Church. You will receive a letter with your scheduled pick up time.
