I authorize SARNIA UPWARD BASKETBALL (including SEMC, Bluewater Baptist and Paterson Memorial) , and its staff and volunteers, to obtain medical assistance in emergencies.

I take personal responsibility and agree to hold harmless SARNIA UPWARD BASKETBALL (including SEMC, Bluewater Baptist and Paterson Memorial) , its Leaders and Program Personnel from and against any loss, damage or injury suffered by me as a result of being part of the activities of SARNIA UPWARD BASKETBALL, as well as of any medical treatment authorized by the supervising individuals representing SARNIA UPWARD BASKETBALL. This consent and authorization is effective only when participating in events sponsored by SARNIA UPWARD BASKETBALL.
