Lakeshore Sport & Fitness is adding a shuttle service for all 4th-8th graders from Burley School.  Our labeled LSF Express will sit at a designated meeting location (see below) from 3:15-3:30pm Monday-Friday.  The bus will bring them back to LSF so they can participate in Club 48 or other scheduled activities.

Meeting location: Students will cross Ashland at the corner of Barry and Ashland with the crossing guard and meet our LSF team member at the pay lot in the middle of the block on Barry between Ashland and Lincoln.  They'll look for the big orange & black van!

It is required that parents complete the following prior to using the service:

1) Complete the form below.  We must know to expect your child on the bus; if your child is not registered, we have the ability to turn away service.  We must know how many kids to expect daily, as our bus capacity is 14 riders.  If your schedule changes, please communicate this with us via email to Club48@LakeshoreSF.com.

2) CPS requires a signed liability waiver.  Please see this form at the bottom of this form.  Print, complete and email back to Club48@LakeshoreSF.com.
