Homecenter is the after school program for students of LaGrange Academy, available to grades Pre-K - 5th grades. This is available to use full time or part time throughout the week. Please communicate directly with your child's homeroom teachers regarding days of the week to send them to Homecenter. Read through the policies noted below and submit this form.

FEES:

Fees will be billed at the rate of $15.00 per day per child. If your child is in Home Center for any portion of the day you will be billed for the entire day. Home Center fees will be billed through the office on a monthly basis. A $1.00 late fee will be billed for every minute that you are late picking up your child. Homecenter ends promptly at 5:30 pm.

EMERGENCY CONTACTS AND AUTHORIZED PICK-UP:

It is the parents' responsibility to keep a child’s records current in Renweb to reflect any significant changes as they occur, such as phone numbers, work location, emergency contacts, child’s physician, child’s health status, list of approved transportation, etc.

Parents, (or person authorized), must accompany their children from the center. Children will not be allowed to arrive or be dismissed from Homecenter without being escorted by the parent, person authorized by the parent, or school personnel.

If anyone other than the designated person is to pick up the child, the center must be informed of the change on that day.

PROTECTION OF CHILDREN IN THE FACILITY:

In the event of severe weather, fire, gas leaks, etc. a plan for evacuation will be posted in each area. The children will be made familiar with this procedure. The center is a smoke-free facility at all times.

School personnel is required to report any suspected case of child abuse, neglect, exploitation, or deprivation to the Department of Family Services.

GUIDANCE AND DISCIPLINE TECHNIQUES:

Attendees will be given an opportunity to complete homework. It is the parents' responsibility to check homework and make sure it is complete and correct.

If a child is not adhering to the Home Center rules and continues the behavior after being spoken to, he/she will be asked to sit out for a time. If the behavior continues beyond this the parents will be called to pick up the child. The Center reserves the right to refuse to let the child return if there are repeated disciplinary incidents.

SNACKS AND FOOD EXCEPTIONS:

Home Center will provide an afternoon snack. If there are any food exceptions for food allergies these must be provided to the center in writing by a doctor.

EMERGENCY MEDICAL CARE, ILLNESS AND INJURIES:

Homecenter personnel will not have access to OTC medicines once the main office is closed for the day. Personnel will not be responsible to administer any prescription medications.Therefore, In case of illness, the child will be removed from the group to an area where he/she can rest. A child will be excluded if he/she has a temperature of 100 degrees or higher or other symptoms such as diarrhea, sore throat, and the parent will be contacted immediately to pick up a child.

Minor injuries will be treated and the parent informed when that child is picked up. In the case of a medical emergency, emergency medical services will be contacted immediately and the child will be transported to West Georgia Medical Center. The parents will be contacted immediately.
