2020 Faith Formation Registration Form
If the student who is being registered is being raised by a legal guardian, do not use this form. Please download the printed form and register in person with Jane Santos of Beth Hoye so that the guardian situation can be fully and confidentially explained.  

If this is a new registration, then you must have a copy of your child’s baptismal certificate if they were baptized outside of St. Anthony, St. Jude, or St. Mary.

We are no longer using Paypal for on-line payment. Instead we are using our regular Parish on-line payment system, WeShare, which is found on the collaborative website, cntaunton.org.

In grades 1, 3 & 5 we are going to do at-home learning using the Pflaum Gospel weeklies. You can receive the it either on-line from the Pflaum web site, or a printed copy with a few weeks in a packet. Which do you prefer?
