This is a request form for a single event reservation for parties of 15-52. If you are interested in hosting multiple events - please fill out a separate form for each. A team member from Mani and Isalita will contact you to confirm your reservation.
Please note:
-- Once your reservation is confirmed, our team will send you an Event Policy Agreement; it must be read and signed by the host

-- Deposit and contract will be required to finalize reservation

-- Please note, reservations are non transferrable

-- All private and semi-private rooms will have a fixed price with a curated menu and beverage package

-- If you are interested in multiple dates, please fill out a separate form for each

-- While we will do our best to accommodate all requests, reservation request forms made in error that are either incomplete submissions or mistakes to time/date/guest count/etc. will be viewed only after all correctly submitted requests have been reviewed and accepted.

In order to be fair to all those looking to celebrate, all events will be contracted for 2 hours.
