Sewing Guard Mask Request Form - 40th Ward

The 40th Ward Sewing Guard is an amazing grassroots network of sewers making reusable cloth masks for the COVID-19 crisis. We are grateful for their hard work every day!

If you are interested in receiving reusable cloth masks, either as an individual or a business, nonprofit, etc., and are unable to obtain one through other means, please fill out this form, and we will arrange delivery to your home as quickly as possible.

Due to high demand, it may take 1-2 weeks to complete mask orders. IF YOU HAVE ACCESS TO OTHER SOURCES, PLEASE CONSIDER HOLDING OFF ON YOUR REQUEST -- we want to prioritize our homebound, senior, and movement-restricted residents first for home delivery.

(Please note, on this form you can enter "none" in any required field if you do not have an email address, phone number, etc. However, we cannot deliver masks without an accurate street address, and we can only deliver to 40th Ward residents and organizations!)
