LNHS Transcript Request
THIS FORM IS NOT FOR WITHDRAWALS.  IF YOU NEED A TRANSCRIPT FOR A WITHDRAWAL, YOU MUST COME TO THE SCHOOL AND FILL OUT THE WITHDRAWAL FORM AND PROVIDE A COPY OF YOUR ID.

Please use this form to request transcripts. There is no fee for electronic transcripts. There is a $2.00 fee for printed transcripts. Printed transcripts will be available in the front office. You will be notified by email when transcripts have been sent or are ready for pickup. TRANSCRIPTS CANNOT BE EMAILED.  Questions about transcript requests should be sent to  willmary.moralestorres@ocps.net.

Transcripts will be processed 24-48 hours after receiving the request. If transcripts are not collected within 30 days, they will be discarded.
