Docuphase New User Request or Changes Form
Supervisors should use this form to request a new user account to use Docuphase (formerly called Treeno) and for any changes to permissions for access to particular cabinets or folders.  If the new employee is replacing an existing employee, he/she will be assigned the same permissions as the employee being replaced unless additional permissions are requested and approved.  Access to confidential folders for students and employees must be approved.

Employee, Student, and some confidential cabinets are locked and require approval by the Director of HR and/or the Director of Business Services before permissions can be activated for these two restricted confidential cabinets depending on your assigned role.
