The Makah Tribe has taken a strong lead in preventing the loss of life during the COVID-19 Pandemic.
Per Health Order 04-21, Enrolled Makahs and their household members traveling with them are allowed to visit the Makah Indian Reservation without an Attestation or proof of vaccination. This form is for *non-Makahs who do not live with an Enrolled Makah Tribal Member*.
Non-Makah Personal Visitors who are Fully Vaccinated and provide proof via a Vaccine Record Card and a matching government-issued identification card may apply to visit the Makah Reservation. Household members of Personal Visitors who are not yet eligible by age for vaccination may be allowed to accompany their parent/guardian on to the Reservation and must follow the Visitor Protocols.
Personal Visitors and the Neah Bay residents that the Personal Visitor is visiting must also be Fully Vaccinated (unless they are not eligible for the vaccine because of their age). Proof of vaccination plus a government identification card must be emailed to
info@makah.com.
This Personal Visitor process is in effect as a trial period from July 12 - August 20.
Applicants and their hosts will be contacted within 2 business days by phone. If no contact information is provided, the applicant will automatically be denied access. If you do not receive a phone call, please call Brittany Olson or Tara Barros at 360-645-2201.
Business Visitors must complete the Business Visitor Attestation form located at
stihc.org. Business Visitors should not fill out this form.
