I am signing up to attend the Oregon Shakespeare Festival trip to Ashland, OR

To hold my spot, I understand that I need to pay a $75 deposit which is NOT refundable. A Wayfinding Academy crew member will email me after I submit this form to help me do this.

I agree to pay the cost of the trip by Sept 13, 2019.

If an unforeseen emergency occurs while on this trip, I agree that there may be an additional cost to get me or my fellow participants to safety. Wayfinding Academy will work with me to pay this cost if this should come up.

I will read and sign a Learn and Explore trip waiver and complete a basic medical form before leaving for this trip. A Wayfinding Academy crew member will give me more information about this after I sign up.

I agree to participate in at least one pre-trip meeting either in person in Portland, OR or remotely via Skype or other technology.

I have read the Wayfinding Academy Code of Conduct (found at: http://bit.ly/2flv9gl) and agree to the pledge found on page 4.
