Please review the 5 DAY Kindergarten Program informational letter.

Agreement Provisions:

- You must submit this form to formally register your child for the 5 Day Program.

- To secure a space in our optional 5 DAY Kindergarten program,  there is a $100 deposit required. Parents have the option of mailing a check or calling the Treasurer's Office.
  - If mailing the deposit, mail to 4122 N Linda Dr, Bellbrook, OH 45305
  - If paying by credit card, please call Jessica Cain at 937-848-5001 x 6112 on Mondays / Wednesdays between
    the hours of 9am - 12pm

- You must upload a copy of your student's birth certificate, immunization records and 2 proofs of residency to your student's Final Forms documents folder.

- Please submit this form ASAP. Pending enrollment, the classes may close at any time.  

- The tuition for this 5 DAY Kindergarten Program is $3,600.00 payable in 9 monthly installments of $400.

- Monthly Fee payments must be mailed to Bellbrook-Sugarcreek Schools Treasurer’s Office, Attn: Jessica Cain, 3757 Upper Bellbrook Road, Bellbrook, Ohio 45305.

- The August payment is DUE at your child’s August Orientation Appointment or they may not start school in this 5 Day Kindergarten program.  Monthly payments are due on the first day of each month beginning in September 2022 and ending in April 2023.

- Your monthly tuition does NOT cover the cost of field trips, school supplies, student fees, and daily lunches.

- Any child whose tuition is not paid in a timely manner may be excluded from the Daily Kindergarten Program.
