Open to: Boys Grades 1st-8th who plan to attend Biddeford High School in the future 
Date: Sunday November 3rd
Time: 8:45am - 11:30pm (Check-in starts at 8:30am, front door will not be open until 8:30am)
Location: Tiger Gym at Biddeford High School
Cost: Suggested donation of $5 (all proceeds go to our Biddeford Hoops youth program which provides travel basketball opportunities beyond recreation and middle school basketball)
Payment:       Please bring cash or check written to: "Biddeford Hoops" on the day of the clinic 
Registration Deadline:  Friday November 1st 12:00pm

Parental Consent: I understand there are risks of physical injury in participating in sports and  recreational activities or programs. I hereby release the City of Biddeford, its employees, officials and agents  from any and all liability, loss, or damage to personal property that my child or I may experience in connection with activities sponsored by Biddeford Hoops. I hereby consent to emergency medical procedures deemed  advisable for my child in the event I cannot be reached and my child has sustained an injury. The school does not provide accident or hospitalization insurance for participants of its programs. All participants are advised to  have adequate personal coverage. I also consent to the use of me or my child’s photo, video, artwork etc. by the dept for flyers, presentations etc.
