1. I give permission for my child to participate in the Lawrence Travel Soccer Program sponsored by Beyond Soccer. I also authorize my child to participate in Beyond Soccer's off the field programs, including its related health and wellness programs (Health Kicks! – i.e. speed and agility workouts, cooking courses), leadership programs, and special group outings (i.e. leadership conferences, community service events, pasta parties, after- professional sporting events). **Please note that any of these opportunities will follow COVID-19 protocols and are not expected this spring 2021 season.

2. This registration form covers the period of March 30th - June 15th (2021) Separate permission forms will be required for special summer camps.

3. I hereby certify that my child is in normal health and capable of safe participation in youth sports programs. I understand and waive related liability claims for myself and/or my child and assume all customary risks and hazards incidental to the reasonable operation and conduct of this program and for the necessary transportation to and from the program, when and if provided for by Beyond Soccer staff or transportation contractors.

3. Por la presente certifico que mi hijo está en salud normal y es capaz de participar de manera segura en programas deportivos juveniles. Entiendo y renuncio a reclamos de responsabilidad relacionados para mí y /o mi hijo y asumo todos los riesgos y peligros consuetudinarios incidentales a la operación y conducción razonable de este programa y para el transporte necesario hacia y desde el programa, cuando y si así lo proporciona el personal de Beyond Soccer o contratistas de transporte.

4. I hereby authorize Beyond Soccer to obtain medical treatment for my child if I (parent/guardian), or my child's emergency contact, cannot be reached. This authorization, intended for emergency use only, and includes the authority to consent to any x-ray examinations, anesthetic, medical procedure or treatment, and hospital care under the supervision, and upon the advice of or to be rendered by, a licensed physician, nurse, or dentist for my child.

4. Por la presente autorizo a Beyond Soccer a obtener tratamiento médico para mi hijo si yo (padre/tutor), o el contacto de emergencia de mi hijo, no pueden ser contactados. Esta autorización, destinada únicamente a uso de emergencia, e incluye la autoridad para dar su consentimiento a cualquier examen de rayos X, anestésico, procedimiento médico o tratamiento, y atención hospitalaria bajo la supervisión, y bajo el consejo o para ser prestado por un médico con licencia, enfermera o dentista para mi hijo.

5. I hereby give my consent for Beyond Soccer to use my child’s photograph and likeness in its publications (newsletter), including its website www.beyondsoccerlawrence.org, Facebook, Instagram, and Twitter page. I release Beyond Soccer from any claim of violating privacy or confidentiality rights that I could assert for the above minor child and myself and attest that I am the parent or legal guardian of the child listed above.

Por la presente doy mi consentimiento para que Beyond Soccer utilice la fotografía y semejanza de mi hijo en sus publicaciones (newsletter), incluyendo su sitio web www.beyondsoccerlawrence.org, Facebook, Instagram y twitter. Libero a Beyond Soccer de cualquier reclamación de violación de los derechos de privacidad o confidencialidad que pueda afirmar por el niño menor de edad anterior y por mí mismo y atestiguo que soy el padre o tutor legal del niño mencionado anteriormente.
