This form should be completed by anyone not currently on any panel and seeking admission. Those on a panel and seeking additional panels should complete the Status Change Application instead.

Understanding the Application Process

Before beginning, you should gather the following:

A list of your most recent and/or serious jury trials including defendant, cause, charge, court, date, outcome, etc. Please be familiar with the minimum qualifications for the panel you are applying to and be ready to provide the necessary information for that panel.

A list of your most recent and/or serious mental health cases (if applying to a Mental Health Panel).

List of recent appeals (if applying to an Appellate Panel).
