Before filling in this form, please be aware of the following:

- In accordance with our premises license, we do not allow parties for 18th/21st birthdays.
- You must only be in the building for the hours that you have paid for, because 1) we may have booked in other hirers before/after you and 2) you are only insured for the hours you have been invoiced for.
- If your function will include the sale and/or consumption of alcohol, then you must inform us. The Parish Council reserves the right to refuse your event

Do you require additional set up of the rooms - eg tables and chairs laid out? This will be an additional cost (usually 1 hour room hire fee). If so please tell us what you would need.
Hiring discounts - do you qualify? *
We offer a reduced rate to residents of Wheathampstead (AL4 8) and regular hirers (after an agreed period of time). Please confirm which category you fall into so we can invoice you accordingly. This is best discussed with us over the phone prior to completing this form if possible.

Will your event include the consumption of alcohol? Please specify below. If you choose the first option, the Designated Premises Supervisor will be in touch to discuss the next steps. If alcohol is sold without our knowledge or permission, you will be in breach of our Premises License and risk incurring a penalty of up to £2,000. Full details can be found on our website.

By ticking the box below, you are confirming that 1) you have READ AND ACCEPTED the Conditions of Letting (found on our website), 2) you agree to pay a 20% deposit within 7 days of receipt of the first invoice, plus a refundable damages deposit of £50 or £100 (see section 3.0 of the Conditions of Letting), 3) you agree to pay the outstanding balance within 7 days prior to your booking date, 4) you agree to complete a supplementary licensing form if alcohol is to be sold during the event and 5) you are over 18 years of age.
