This form is for requesting gender-affirming care supplies with no out-of-pocket costs to you. This is a combined effort of the
Office of Pluralism and Leadership (OPAL), the health service at
Dick's House, and the
DHMC Pharmacy at Dick Hall's House.
Currently enrolled Dartmouth students attending classes in Hanover are eligible to select up to two items per term, free of charge. It is important to note that multi-pack offerings from retailers are not considered as a single item. We appreciate your understanding.We are not a big online retailer, we know this is a little clunky but we are trying.
