Please fill out the details below and either make your payment of $50 online (details below) or bring cash to pay Donald on the night.  Donald will also issue receipts for online payments on the night if anyone wishes them.  Reservations must be received by 24 February for catering purposes.

If you find yourself unable to attend at the last minute, I'm afraid we will still come calling for the dinner fee.  It is fine to cancel up to a week prior to the dinner, but we cannot, regrettably, afford to carry the costs of last-minute cancellations.
