YDP 2024 Summer Field Trip Registration and Permission Form
PLEASE FILL OUT THE PROGRAM REGISTRATION FORM BEFORE FILING OUT THE SUMMER FIELD TRIP REGISTRATION AND PERMISSION FORM

We are thrilled to offer an exceptional opportunity for the participants of the Filipino Community of Seattle's Youth Development Programs. As part of our commitment to enriching the curriculum and providing an enhanced educational experience, we have carefully planned a series of field trips to various destinations. These trips are designed to broaden horizons, foster cultural understanding, and create lasting memories. We kindly request your permission for your child to participate in these valuable excursions. Before registering for our field trips, please make sure your child is registered for our Youth Program. Field trips are only free for those who are participating and available for active participants. If your child is not yet registered, please sign up at filcommsea.org/youth-development

To ensure a smooth and organized process, please follow the steps below to confirm your child's participation:

Step 1: Review the provided field trip information in detail. Step 2: Complete the student information section with accurate and complete details. Step 3: Fill out the parent/guardian signature section in full. Step 4: Initial the participation permission for each field trip you approve. Step 5: Return or submit the completed form at least one week before the first field trip you would like to sign up your child for.

We want to assure you that all field trip transportation will be facilitated using the vehicles of the Filipino Community of Seattle, driven by licensed and background-checked individuals. In the unlikely event of any changes to this arrangement, we will promptly notify parents/guardians and provide a transportation waiver at least one week prior to the field trips.

Additionally, please note that under each field trip listed, you will find important information about attire and lunch arrangements. Please arrive an hour before the field trip times listed at our center (5740 Martin Luther King Jr. Way S, Seattle WA, 98118). As each field trip has limited spots, we will call before each field trip to confirm if there is space for your child. Please note that spots are on a first come first serve basis as well. We will send confirmation emails and make reminder calls to keep you informed.

All field trips are free of charge, as part of our commitment to making these experiences accessible to all families. We are eagerly looking forward to your family's participation in our Batibot and Bagets youth programs this summer at FCS. Together, let's create unforgettable moments of learning and growth.
