Use this form to order any of the following items from the 2020-21 school year:
- Team picture as 8x10 or 5x7 print with a custom background and the CCS sport logo,
- Individual student/athlete picture as 8x10 or 5x7 print with a custom background,
- Combination 8x10 print including both the team and the student with a custom border, and
- A thumb drive containing all ACTION shots taken of your student as displayed on www.JimLisaPhotography.com

Within days of completing this form, you will receive an invoice from Jim & Lisa Photography LLC (service@paypal.com) that will include a link to a proof of the print(s) ordered or a gallery of the action shots. If the order is correct, you will have the opportunity to purchase the item(s) with an online payment; if you prefer to pay by check or cash, you can print the invoice and mail it with payment to Jim & Lisa Photography or send it in to the High School or Middle School desk at CCS. The products will be delivered as instructed after payment is received.
