Hello! This is the QMed Accessibility Feedback Form, as part of the new AS Accessibility Policy.

Please fill out this form if you have any feedback relating to the implementation and/or details of the Queen's Medicine AS Accessibility Policy (available here, click on 'Aesculapian Society Accessibility Policy': https://meds.queensu.ca/qmed/council/constitutional-documents).

This form can be filled out throughout the year - and will be frequently checked after QMed events.
