Prior to filling out this form, the following criteria should be met

Both teams must be 100% assured of their playoff match-up game.
Both teams must give verbal notice to the Burkburnett ISD Assistant Head Coach, Patrick Williams, as soon as they agree to play. Download and read the specific guidelines for the facility needed. Please reference the Burkburnett ISD Facility Use Packet on the BISD website for facility rental.
