UHS Virtual Tutoring Center - Parent Consent for Live Video Conferencing
Please read the consent statement below regarding Live Video Conferencing using Google Meet for the UHS Virtual Tutoring Center.  Simply reply on this form from a parent/guardian email address to give permission for your child  to participate in the UHS Virtual Tutoring Center.

I give my consent for my child to engage in Live Video Conferencing with the following UHS staff: Ms. Anderson, Ms. Martin, Ms. Teti, and/or Ms. Svincek.

I understand that while video conferencing has potential benefits, including direct access to the classroom teacher, live explanations, participation with other classmates etc., there are potential risks, including interruptions and technical difficulties.  Also, I do not assume that the educator has immediate access to any or all of the technical information used for video conferencing, and may need to end a session and consult technical services when necessary to address technical difficulties.

Please do not record virtual video special education instruction or services, telepractice or telehealth, during the implementation of Individualized Distance Learning Plans (IDLP), unless doing so is required for Free Appropriate Public Education (FAPE) and is identified on the student's IDLP.

While your child may have been using Google products prior to distance learning and the expansion to Google Meet, the Google Terms of Service have been updated and are provided herein using the following link.
