INSTRUCTION

2. ENTRANCE EXAMINATION is not required in the enrollment process for this school year only. However, the student will undergo the Diagnostic Examination by the subject teachers when classes start to assess the level of academic competency.

B. New Student (to be submitted through email in the email provided, the hard copy can be submitted in the office anytime until September 2020)
          1. Senior HS Report Card or Form 138 (Original and Photocopy)
          2. Good Moral Certificate (Original and Photocopy)
          3. Birth Certificate (PSA-Original and Photocopy)
          4. ID Picture with white background

          5. 2 pieces Long Brown Envelope

C. Transferee Student
          1. Certificate of Transfer Credential (Honorable Dismissal)/Certificate of Good Moral Character
          2. Transcript of Record (TOR)
          3. ID Picture with white background

          4. 2 pieces Long Brown Envelope
