Multi-dog households - The questionnaire is specific to one dog. However, I do provide the opportunity to detail other pets you may have. If your struggle(s) is/are with multiple dogs, please ensure you provide details of all pets somewhere on the form. 

Once you have submitted the form, I will contact you as soon as possible to confirm my availability. Please bear with me while waiting for a response, I am very busy at the moment. 

How your data is used: Through the registration process the form will collect your name and contact details including email address, phone number and address. I also collect information about your dog including their breed, age, name and behaviours. This helps to inform your training plan. I may collect photographs and/or videos of the training you do with your dog. If you are not comfortable with us sharing these, please contact me at dogologicaltraining@gmail.com. Where you provide feedback, this may be used publicly e.g. Facebook or website. If you are not comfortable with this, please contact me on  dogologicaltraining@gmail.com .
