SarcoidosisUK Neurosarcoidosis Patient Day - Networking Interest Form

You have reserved a ticket for the SarcoidosisUK Neurosarcoidosis Patient Day on June 15th. Thank you for your support! We are really looking forward to meeting you and bringing together people across the UK affected by neurosarcoidosis (NS).

Before our programme of speakers begins at 2.15pm, there will be a chance to meet other attendees. We think there will have never been so many NS patients all together in one room! This will be an amazing opportunity to meet others like you and to share ideas, best practice and support each other.

If you are interested in participating in some informal networking from 1pm until 2pm before the main event starts, please fill in the form below. Your answers will give us an idea of who you are and how you are affected by NS. We will then create small groups to make sure you are connecting with others with whom you have something in common (e.g. geographical location, type of sarcoidosis, consultant).
