Samba Class Registration
Each class will explore the fundamental samba movements and expression. No experience necessary. 
  • WHEN: Wednesdays (April 24-May 29)
  • TIME: 7:00-8:00 PM
  • WHERE: The Breathing Room (918 Bridge St NW, Winston-Salem, NC 27101)
  • COST: $15 per class
What to wear: Comfortable workout clothes and tennis shoes. No heels needed. 
What to bring: A water bottle and good energy!

Unfortunately, there are no refunds for missed classes.

I understand that participation in dance classes and activities could involve some risks. Despite precautions, accidents, injuries, and illness may occur. By signing this release form, I (the dancer) assume all risks related to the use of any and all spaces used by Courtney Feliz. 

I agree to release and hold harmless Courtney Feliz including teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I will not hold Courtney Feliz liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I agree to obey the class and facility rules and take full responsibility for my behavior in addition to any damage I may cause to the facilities utilized by Courtney Feliz. In the event that I should observe any unsafe conduct or conditions before, during or after my classes, I agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the associated risks. I have signed this document voluntarily and of my own free will.
