DISCLOSURE STATEMENT AND AGREEMENT FOR SERVICES
This agreement is intended to provide important information to you regarding Life Coaching Services.
Please read the entire document carefully and ask your Coach any questions you might have regarding
its contents.

ABOUT THE COACHING PROCESS
A Life Coach works with you to achieve your personal, academic, financial, relationship, and business goals. This is usually a one-on-one process, but is helpful for achieving relationship goals as well. A life coach will discuss what is happening in your life now and your experiences with this topic, help you minimize any limiting beliefs and obstacles you might be facing, uncover and explore your personal strengths and resources, aid in personal growth and empowerment, and help you develop a personalized plan of action that works for you. Your plan of action is based on your skills, your experience, your beliefs, your resources, your desires, and your life. Life Coaching is NOT mental health counseling. Your life coach is focused on where you’re currently at and helping you move in a fresh direction for a better future. Coaching is solution-focused and recognizes that Clients are the experts in what works best for them. You and your coach will work collaboratively to reveal what's missing, what works, where and who you want to be, and how to get there.

ABOUT YOUR COACH
Cheryl McGill holds a Master’s degree in Clinical Mental Health Counseling and is a Certified Life Coach (Life Coach Training Institute Certification#: Gcawrngaxk). Cheryl is currently pursuing a PhD in Holistic Life Coaching at University of Sedona.

COST OF SERVICES
Each Life Coaching session is $125. Life Coaching services are not covered by health insurance, but may be authorized by select Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA). Payment is due at time of service.

CANCELLATION POLICY
24 hour advance notice is required should you need to cancel your appointment. Late cancellations will incur a fee equal to one-half of the Life Coaching session fee ($62.50). Not attending your appointment without cancelling will incur a fee equal to the full Life Coaching session fee ($125).

TERMINATION OF THE LIFE COACHING RELATIONSHIP
The Coach or the Client may discontinue Coaching at any time. The length of your Coaching will depend on the specifics of your goals and the progress you achieve. Your Coach will help define your goals, assist in momentum toward those goals, and provide feedback to stay on track toward achieving your goals. However, it is the Client’s responsibility to create the time and energy to fully participate in the Coaching process.

CONFIDENTIALITY
Your Life Coach is bound by the International Association of Certified Coaches (IACC) Code of Ethics to protect the privacy and confidentiality of Client information. However, disclosure may occur if your Coach is required by statute, lawfully issued subpoena, or by court order to disclose or if information disclosed during session leads your Coach to reasonably believe there to be an imminent or likely risk of danger or harm to the Client or others. Finally, topics may be anonymously and hypothetically shared with other coaching professionals for training, supervision, mentoring, evaluation, and for coach professional development and/or consultation purposes.

LIABILITY
Your Coach makes no guarantees, representations or warranties of any kind or nature, expressed or implied, with respect to Coaching services. Your Coach is working only within the capacities of a Certified Life Coach and will not be held liable to the Client for any indirect, consequential or special damages. Notwithstanding any damages that the Client may incur, the Coach’s entire liability under this Agreement, and the Client’s exclusive remedy, shall be limited to the amount actually paid by the Client to the Coach under this Agreement for all coaching services rendered through and including the termination date.

DISCLAIMER
Life Coaching is not counseling or psychotherapy and is not appropriate for the diagnosis or treatment of mental health disorders. You are advised to seek treatment from a Licensed Professional Counselor or Psychologist for such disorders. This agreement is for Life Coaching and you agree that you understand the difference in Coaching and counseling/psychotherapy.
