Hello, and welcome to the Toronto Psychology Clinic – we're here to support you on your mental health journey! This intake form is designed to help our caring Intake team understand more about your unique therapy needs.

We understand that everyone's comfort level is different, so if filling out forms isn't your thing, no worries! Feel free to shoot us an email or leave a voicemail with your name, contact details, and the best time for us to reach out.

By completing and submitting this form, you consent for us to collect, store, and share your personal health information with our TPC administration and therapists per our Terms and Conditions and Privacy Policy (listed on our website torontopsychology.com) *
We DO NOT provide: Legal assessments (e.g., court-mandated or immigration-related, etc. assessments), ADHD, Autism, and Neurological assessments/diagnosis, WSIB or MVA assessments or treatment, or substance use problems that are the primary concern.  *
We DO NOT provide direct billing to insurance companies. We will provide you with receipts that you can submit to insurance for reimbursement. *
Our services are NOT covered by OHIP. We currently DO NOT provide sliding scale fees. Our fees for therapists are as follows: $275-$320 for a Registered Psychologist/Psychological Associate, and $180-$226 for a Registered Social Worker/Psychotherapist. *
Wait times for an initial appointment can vary from 2 weeks to a few months, depending on the therapist you are requesting or who is the best fit for you.  *
