I, the undersigned, individually or as a parent/guardian of this applicant, a minor, ask that he/she be admitted to participate as a volunteer for Camp Nature Odyssey. In consideration of such admission, I do hereby agree to release, discharge, and hold harmless the Passionist Earth & Spirit Center, their officers, agents, volunteers and employees of and from all causes, liabilities, damages, claims, or demands whatsoever on account of any injury or accident involving the said minor arising out of the minor’s attendance at Camp Odyssey or in the course of activities held in connection with Camp Odyssey. By signing this agreement, I also acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I, or my child, children or a minor for whom I am the legal guardian (collectively “my children”), may be exposed to or infected by COVID-19 while participating in Camp Odyssey, and that such exposure or infection may result in injury, illness, permanent disability, and death. On my behalf and on behalf of my children, I agree to assume these risks and accept sole responsibility for any injury to myself or my children including, but not limited to, bodily injury, disability, death, illness, damage, loss, claim, liability, or expense, of any kind, that I and my children may experience or incur in connection with my or my children’s attendance at and/or participation in Camp Odyssey (“Claims”). On my behalf and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Passionist Earth & Spirit Center, its officers, agents, volunteers and employees of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.  Type own name or parent/guardian name here (if applicant is minor).
