Please complete this form each time your child is absent from school. Absences for vacations during the school year are discouraged, and must be communicated to the teacher in advance. Advance notice is also appreciated for all scheduled appointments. If you have any questions or if you need to convey more information, please contact Mrs. Anderson at the school office, 507-523-3143 / eanderson@immanuelsilo.org. All absences should be reported no later than 8:15 am by using this form.
