2019 Cortez BMX Jersey Order form

There will be two chances to order custom Cortez BMX jerseys for the 2019 racing season:

1) Early Bird Special April 2nd-April 31st: Long sleeved jersey $40; Short sleeved jersey $38.

* This order will be submitted on May 1st in order to have new jerseys before our State Race on June 9, 2019.

**Cortez BMX must have payment in full before May 1st or we will not be able to place your order.**
You can make payment at the BMX track during our track maintenance days (April 12-13 and April 26-27) or mail a check (make checks out to "Cortez BMX")

2) A second order will be placed in early June: Pricing will be $45 long sleeved and $43 short sleeved
