When do I fill out this form?
1- When I am going to be out for a block or two and have found coverage for my time out
2- When I am going to be out for a block or two and have not been able to find coverage for my time out- if you have tried and not been successful then staff will be assigned to the block

When to use S4T/ Frontline
1- When I am taking leave for the day- request a sub in all cases
2- When I am taking leave for half a day- request a sub in all cases
