ALL FORMS ARE VALID FOR 30 DAYS FROM THE DATE OF SUBMISSION.

WE WILL PRINT YOUR FORM IN  THE OFFICE FOR YOU - PLEASE DO NOT PRINT THIS FORM AND BRING IT WITH YOUR SAMPLES.

MCSWCD uses A&L Laboratories in Ft. Wayne, Indiana for sample processing unless otherwise noted.  We will request recommendations from A&L Labs.  The MCSWCD staff will be glad to discuss your soil sample with you to the best of their ability's, but MCSWCD staff are not Soil Scientists.  Thus MCSWCD will not be held liable to any changes or amendments you might choose to make in your soil.
