This form contains questions identified by Oakland's Fund for Children and Youth (OFCY). OFCY has funded Leadership In Diversity since 2016. As with all it's public and free programs, OFCY requires demographic data to ensure that Oakland's funds and services are reaching students directly and communities with the high need.

The completion of this form is critical for your student to participate in LID for the 2020-2021 school year.

This form is divided into 3 sections. Sections 1 & 2 can be completed by student directly or on behalf of student by their adult, but section 3 MUST be completed and confirmed by the Parent/Guardian.
