I understand that DREALEE | Fitness + Dance located at the Aspen Plaza is not liable for personal injuries, loss of, or damage to, personal property. I understand that participating in the classes at DREALEE | Fitness + Dance involves the possibility of injury. I am participating voluntarily and all risks of the classes have been made clear to me. I do not have any conditions that will limit my involvement in the class. I agree to disclose any disabilities. I have notified management if I am pregnant or get pregnant. I exempt all claims, loss or physical injury in my participation at DREALEE | Fitness + Dance. 

By checking yes below, I have read and agree to the above as stated on this liability waiver. I forfeit all right to bring  a suit against DREALEE | Fitness + Dance for any reason. I agree to hold DREALEE | Fitness + Dance free  from any liabilities, including financial responsibility to injury. In return, I will participate in the classes  offered at DREALEE | Fitness + Dance. I will also make every effort to obey safety precautions as listed in  writing and as explained to me verbally. I will ask for clarification when needed. 

I give media use rights permission. (This includes but is not limited to: photos, video, website,  promotional materials such as brochures or banners, YouTube, Vimeo, Instagram, Facebook, etc).
