Fall 2024 Public Health Force Add
To submit a force add request for a Public Health course, please answer all the questions below. Students must answer every question for your request to be submitted. Students can only request one force add per form. If you are requesting to be force added into multiple courses, please submit 1 form for each course.

Student trying to force add courses that are not PHS courses, should check the universal force add direction info sheet and locate the department to learn about their force add process.
