Please use this form to document your arrival and departure from the building to ensure adherence with the PPS COVID-19 Safety Plan any time you access a school building. Before entering a building, employees and contractors are required to view and complete the safety plan training course in Pepper under the title "COVID-19 Safety Plan Training Course" (Pepper Course Number: PES1001).

Building principals and/or designee/s are responsible for monitoring the safety plan which includes daily the sign in and sign out process, and ensuring everyone is social distancing and wearing a mask. This survey has been created to be used to comply with the Oregon Health Authority and PPS expectations. Please direct any questions or concerns to your site admin.
